We want to ensure that your registration process is completed as quickly as possible.

Centralized Scheduling means all procedures at Logan Memorial are scheduled through a single phone call. Physicians and patients alike benefit from the convenient, precise coordination of services and availability of accurate preparatory instructions.

Checking in for both Inpatient and Outpatient Services: Registration is located in the front lobby of the hospital from 6:30 AM -4:30 PM, Monday through Friday, and in the emergency room evenings and weekends.

You may be required to sign the following:

• Consent for treatment
  
• Authorization to bill insurance
  

If you are a Medicare recipient, you will be required to sign:

• Medicare Secondary questionnaire
  
• Medicare Rights Statement

Remember to bring with you at the time of registration the following items:

• Social Security Card
  
• All active insurance cards
  
• Picture ID
  
• Complete list of current medications
  
• Living Will, if you have one
  

Patients are encouraged to leave valuables at home to ensure safe-keeping.

Please bring with you personal items: such as a robe, slippers, and toiletries.

All diagnostic outpatients are requested to arrive at the Registration Desk 30 minutes prior to the appointment time.
